How to Apply for the Canada Disability Benefit
The Canada Disability Benefit (CDB) provides monthly tax-free payments to eligible low-income working-age persons with disabilities. Below is a detailed guide on how to apply and what documents you need.
📋 What You Need to Apply
The documents you need depend on whether you’re applying for yourself or on behalf of someone else as a legal representative.
If You Are Applying for Yourself
You may have received a letter from Service Canada containing a unique 6-digit application code. If so, you’ll need to provide:
- Your 6-digit application code (top right corner of the letter)
- Your Social Insurance Number (SIN)
- Your legal status in Canada (citizen, PR, etc.)
- Optional but helpful: Direct deposit info and 2024 net income (line 23600 on your Notice of Assessment)
If you did not receive a letter, you can still apply by providing:
- Your SIN
- Your legal status in Canada
- Optional: Direct deposit info and 2024 net income
If You Are a Legal Representative
You must provide documents on behalf of the applicant, including proof of authority and valid identification.
- The applicant’s SIN
- The applicant’s legal status in Canada
- Your proof of legal authority (court order, guardianship certificate, Power of Attorney, etc.)
- Your government-issued ID (not expired)
- Your and the applicant’s mailing address
- Optional: 2024 net income and direct deposit info
If You Represent an Organization
In addition to the documents above, also include:
- Your organization’s mailing and physical address
- Proof you work for the organization (e.g., employment contract, letter from employer)
- Optional: CRA business number or tax ID

- Complete a printable Canada Disability Benefit application form or the Canada Disability Benefit Legal representative application form
